"I don't think there is a precedent for this kind of thing," said Dr. Arjun Srinivasan of the Centers for Disease Control and Prevention. Health officials and doctors have tracked down most of the 14,000 people potentially at risk for fungal meningitis, blamed for the deaths of 24 people and sickening more than 300.
"This is definitely new territory for us," he said.
The fungus' brown-black color signals an armor that — along with being injected near the spine —helped this mold sneak past the immune defenses of otherwise healthy people, said Dr. Arturo Casadevall, a fungal disease specialist at New York's Albert Einstein College of Medicine.
"What we're dealing with here is fundamentally different" from a typical fungal infection, he said. "This is a bug that most of us don't know much about."
But they're learning fast, piecing together clues that promise some hope.
Doctors are beginning to detail in medical journals the first deaths in this outbreak, and the grim autopsy findings make clear that treating early is crucial, before the fungus becomes entrenched. In one case, a woman died in Maryland after the fungus pierced blood vessels in her brain, leading to severe damage.
People getting treated earlier "seem to be doing OK," with fewer of the strokes that characterized the outbreak's beginning, said Dr. Carol Kauffman of the University of Michigan. She has advised the CDC and co-authored advice in the New England Journal of Medicine on how to handle the complex medication used in treatment.
People who got contaminated steroid shots made by a Massachusetts pharmacy have been told to be on guard for months for meningitis symptoms. But the CDC said Wednesday that the biggest risk for getting sick seems to be within 42 days of receiving one of the implicated back injections.
With the tainted shots recalled in late September, that means the period of greatest risk is nearing an end. And it should help doctors bombarded with calls from the worried determine who most needs a spinal tap to look for the very earliest signs of infection.
"We know the farther out you are from receiving an injection, the lower your risk becomes for developing meningitis or other infections. We want to emphasize that," CDC's Dr. Tom Chiller told a conference call for physicians on Thursday.
Still, public health officials recall a 2002 meningitis cluster linked to steroid injections contaminated with a different fungus; one of those victims got sick 152 days after the shot.
Fungal infections don't get a lot of attention, but they afflict millions around the world, said David Perlin of the University of Medicine and Dentistry of New Jersey, who is studying better ways to diagnose them. Most are skin infections like athlete's foot, but fungi also can cause pneumonia, sinusitis and other problems.
Serious infections tend to strike people with immune systems weakened because of cancer, AIDS or other problems. Fungus-caused meningitis in particular is extremely rare— especially in otherwise healthy people like in this outbreak — and it's "very bad news," said Michigan's Kauffman.
While the more common bacterial and viral forms of meningitis tend to strike quickly with obvious symptoms, fungal meningitis grows very slowly and is hard to diagnose. Few antifungal drugs are absorbed into the central nervous system, limiting treatment options. Plus, human cells and fungal cells have a lot of similarities, making it hard to attack the fungus without side effects, Kauffman explained.
The main culprit in this outbreak is a black mold called Exserohilum rostratum, common in dirt and grasses. Only 33 human infections previously had been reported, mostly eye or skin infections in people with weak immune systems, Casadevall said.
Here's how scientists think it's sneaking into the well-guarded spinal cord and brain of a healthy person:
- The steroid injected near the spine reduces inflammation, one of the immune system's defenses against contamination.
- The mold grows quietly until enough accumulates for it to burrow a tiny hole, or abscess, into the lining of the spinal canal, said Dr. William Schaffner of Vanderbilt University. Reaching the spinal fluid inside offers a direct pathway to the brain.
- The fungus' color signals how intractable it is. Brown-black molds produce melanin, the same chemical that helps human skin tan. It guards against the sun's mold-killing ultraviolet rays — and inside people, it fends off both antifungal drugs and other immune-system attacks, Casadevall said.
The good news: Black mold is treatable with a drug named voriconazole, with far fewer side effects than the older treatment initially recommended when the outbreak began.
Still, Kauffman cautioned doctors to carefully monitor patients because differences in metabolism can make levels surge in the bloodstream, causing hallucinations, confusion, nausea and occasionally liver damage. On the flip side, their bodies may process the drug too quickly to battle the fungus. Plus, voriconazole can interact badly with a list of other common medications.
"It's not clear" how long to treat but at least three months is advised, Kauffman said. It begins with intravenous infusions that are hard to administer outside of a hospital. Then once the patient is stable enough, pills can be used.

Another environmental fungus is also called "black mold": Aspergillus niger. Niger means black in Latin. Aspergillus niger is not common, but people with weak immune system can catch this from plants and grasses since it can reside anywhere outside. Most serious is "invasive pulmonary aspergillosis" which grows black fungal balls in the lungs and can spread to the brain and other organs.
Like Exserohilum rostratum, which is mentioned in this article, aspergillosis is also treated with voriconazole, sometimes for 3-6 months. However, since it is hard to diagnose and must be cultivated in the lab, doctors who are suspicious of fungal infections are advised to start I.V. anti-fungal treatment (such as amphotericin) immediately while waiting for test results. Delay can lead to death of the patient.
IMHO-2730490-Actually the Supreme Court in 2002 stripped the FDA of most of its powers to regulate compounding pharmacies as drug manufacturing businesses.The first produce specific drugs, like the old fashion way, which doctors write prescriptions for, while the other create drugs, then go seek doctors who will prescribe.
Attempts to pass laws in 2002 and 2007 failed as they have lobbyists and lawyers which have continually fought the FDA, which does inspections, files orders for compliance, while fighting to have these companies carry out the required actions. But it has proved very difficult maneuvering through all the different State regulations while going through court litigation constantly. As pharmacies are regulated by each state.
With 7,500 companies now and a 3 billion dollar industry, pharmacy compounding companies have a lot of political clout which still keeps the FDA from making much progress. Until Congress passes laws to allow the FDA to make the regulations, its hands are tied. And don't forget, taxpayers keep screaming about their money being spent for any programs, research and employees hired as well.
There have been other fungus outbreaks previously with deaths. Although the article implies it is new. Earlier this year 33 cases of a fungus eye infection were reported. In 2009 another outbreak of infection occurred with black mold as well resulting in death.
Every day this situation is sounding more and more like a science fiction thriller than real life. I feel so sorry for those poor people who had this done to them. I agree. They keep saying that batch 6 is the batch people need to be concerned with. This is certainly on record at each clinic that sues the drug. The FDA and CDC have recommended that testing commence, but I have not heard of any - in fact what I hear is doctors complaining about teh calls. I wonder how many thousands of doses come from a batch? Still seems as if the involved parties are trying to cover their tails. Unfortunately they are not terribly concerned about those directly involved.
Is there a connection with mold and yeast problems that needs to be explored more?
The FDA is not the only ones that are responsible. States have a part also. Each State should assume more responsibility in insuring that all Companies within their State are in accordance both with State Law and Federal Laws. This means that each Company and their employees are to be held responsible for Safety of all items, including Food Products, Medical supplies, etc. This would also apply down to Farms and their produce.
Both State and Federal Laws must be revise so that arrest and prosecution of parties responsible for such offenses of contaminated food and medical supplies are matter-of-fact. These Laws also must remove any sentence of a lesser offense such as Negligent Manslaughter. In this case, the Owners should be guilty of 1st Degree Manslaughter because they knew the consequences that could follow.

The FDA was not responsible in the improper manufacture and storage of these drugs. They were tainted at a Massachusetts pharmacy that did not follow proper procedure of injection manufacture. They alone are at fault. The FDA and CDC are the ones who helped stop the distribution of these drugs and informed the public. They are the ones who should be thanked for helping control it and tracking down the people who were exposed. Also, are you suggesting that the FDA should check every single medicine bottle and pill and have them tested by the FDA themselves? I'd love to see you cough up extra tax dollars to pay for that. It would also be impossible to do. That's why we have laws to protect people against such issues. That's why we have the FDA at all. People like you are so ignorant it's unfortunate you get to vote.
Being a sufferer of mid grade spondylolithesis (vertebrae displacement), I very nearly went for steroid injections. Thanks god I stuck with core exercises and ibuprofen, or I would be in the count. There is something seriously wrong with the medical industry in this country... If one remembers a time not too long ago when we were told not to buy prescription medication from Canada, as it was a way for AlQueda to terrorize us...
I am in shock about this whole entire problem. I was due to have an injection for my back chronic back pain. I had recently started seeing a different Doctor for pain management, so in discussing treatments he suggested this steroid injection as a stop gap for the pain I have.
The closer it was to the date of the deed, the more nervous I was. Now this was before, right before all the flowers that were grown in the steroid medicine!!! I has decided to call off the injection and go with an alternate of some kind like physical therapy. At my appt. my Dr. seemed dissapointed I said no, but oh well. I now know that someone is looking out for me, Thank-God
Why isn't the FDA routinely inspecting these pharmacies? PREVENTION IS THE KEY. This could have been prevented if the FDA and state agencies inspected these pharmacies or any place that manufacturers drugs.

Last fall, I had several of these injections, (spinal as well as epidural under general anaesthesia). I guess that I am okay, (safe?), since I have not had any adverse affects to this point? Geez, I hope so...
