Only a quarter of Americans infected with the AIDS virus are getting effective treatment, according to a U.S. government report released Friday -- and the youngest patients are the worst off. The numbers could worsen if states don’t broaden health care as called for under the 2010 health reform law, scientists worry.
It’s the first comprehensive look by the Centers for Disease Control and Prevention at who is getting effective care, and it doesn’t paint a promising picture. The findings raise even more alarm bells as study after study presented at the International AIDS Conference in Washington this week show that treatment can help stop the spread of HIV.
“The majority of people living with HIV in the United States are not on antiretroviral treatment, not in stable care,” Dr. Kenneth Mayer of The Fenway Institute and Harvard Medical School in Boston told a news conference. “They need to be in care first and then able to get treatment.”
The study finds that just over a third of HIV patients have steady care -- 34 percent of African-Americans, 37 percent of Latinos and 38 percent of whites.
Younger patients are the least likely to be getting the cocktails of drugs that can keep them healthy and help keep them from infecting others. Just 15 percent of those aged 25-34 had the virus suppressed to desired levels, compared to 36 percent of those aged 55-64. Only 22 percent of young adults were even getting HIV drugs to treat their infection, the CDC found.
There’s no cure for the human immunodeficiency virus that causes AIDS and no vaccine. HIV has killed 25 million since it first started spreading globally in the early 1980s, and more than 33 million people are infected worldwide. About 1.1 million people in the United States have HIV, and the CDC estimates that 20 percent of them don’t even know it.
“We have to continue to raise the alarm,” CDC’s top AIDS official, Dr. Kevin Fenton, said in an interview. “We have to find that sense of outrage.”
The same factors are driving high transmission and poor treatment rates among some U.S. groups: poverty, a lack of access to medical care, and a lack of education about what causes HIV and what people can do about it. Policymakers need to understand that treating people with HIV saves money, Fenton said.
“What we now know is that treating HIV is cost-effective. For every dollar spent, you save $2,” Fenton said.
Fenton said the U.S. should pull out the stops on providing condoms, counseling, testing and treatment.
“We need to ensure that states have policies that support routine HIV testing,” Fenton said. “Clearly, this is going to be more challenging in some states than in others.”
Top AIDS experts in the U.S. say no matter what people may think about the moral implications of some of the behavior that leads to HIV infection, it will benefit everyone to get people tested, treated and counseled about controlling their infection.
“Every state really must enact the Affordable Care Act,” said Dr. Judith Aberg, president of the HIV Medicine Association and an AIDS expert at New York University. “States need to fund HIV treatment and prevention. We need to continue this fight.”
Governors of several states have said they will not expand Medicaid, required by the health care law, because they cannot afford it. The U.S. Supreme Court ruled last month that states can decide whether to abide by that provision. States refusing expansion now include Texas, Florida, South Carolina and Louisiana. Medicaid, the state-federal health insurance plan for the poor, currently does not cover most low-income adults with HIV. AIDS activists say it’s essential to controlling the epidemic to get coverage for young adults with HIV, and at risk for HIV.

Remember Ryan White he contracted aids through a blood transfusion as a child and died at 14. Although blood is now screened for HIV, many people contract the HIV through no fault of their own, say through a philandering spouse.

While I have no doubt about the wisdom of what you are saying, I think we are talking about the real world here; not some imaginary ideal world.
HIV/AIDS is God's punishment for immoral behavior and BIG GOVERNMENT is not the answer. On a completely different topic, we need BIG GOVERNMENT to keep giving handouts to farmers in the mid-west to compensate for the drought that the free market cannot handle and that they didn't save enough for. We also need a huge military with socialst health-care to pre-emptively invade random Middle Eastern countries.
Religious nuts and gay-bashers who are most likely deeply closeted gays aside, there are practical issues that people seem to ignore:
1) HIV/AIDS is not a homosexual disease but is mostly a heterosexual disease. Most of the people coming down with AIDS got it from heterosexual partners (often prostitutes and IV drug users) or from "collateral" exposures such as plasma or white cell infusions, clotting factors, etc that combine blood products from hundreds or thousands of people into one dose.
2) HIV/AIDS is rising among young people and one of the major reasons is very little discussed: the circumcision rate among male babies has dropped dramatically. Circumcision can reduce the heterosexual transmission of AIDS by as much as 60% and some other STDs by as much as 90%.
3) The HIV/AIDS infected have the potential to effect everyone because they form a reservoir of disease that finds its way into people's lives through whatever means is possible. To cut down the threat, you have to cut down on the reservoir --- locally, nationally, and globally or it is difficult to find how it can be prevented in the general absence of a cure.
4) There may be a cure somewhere on the distant horizon. There is the famous "Berlin patient" and a couple of new people who have had bone marrow transplants from people with a genetic mutation that causes the body's immune system to destroy the virus. So, while it is distant and expected to be very, very expensive there may be a cure in a couple of decades.
5) But until there is a cure, the sanest public policy is to educate people about how AIDS/HIV is transmitted, and to make sure that they have access to the appropriate drugs and stay on them. While crazy people and haters and pseudo-christians can't see it, disease threatens everyone, not just mythical "others."

HIV is not AIDS. HIV allows AIDS to develop in immune-compromised individuals. HIV on the other hand was first discovered in primates. How it evolved and began infecting humans is up for discussion. It is a blood born disease, it cannot survive outside of the body for very long if at all. It is true that homosexuals are more likely to become infect however I think that has more to do with condom usage than it does the actual sex part.
I think this because heterosexuals can also become infected through sex. With heterosexual couples though condom usage may be much higher, thus less chance of becoming infected. Not basing that on any factual information, just an observation/hypothesis.

Most HIV infections are sexually transmitted followed by needle sharing behavior. Accordingly transmission can largely be prevented. Those that fail to prevent transmission are responsible to themselves and others they infect and should be executed for doing so. The rest of the population should be quarantined.
Saying you can get HIV thru Innocent means like a transfusion takes the rare exception and calls it the rule for the sake of lying about the virus population. The person who did not get the virus thru sexual contact or drug did not engage in risky behavior and until he does he should not be quarantined. If I'm not mistaken, the AIDS relief in Africa project was President Bush's program. It's a good one. Africa has a tremendous problem with AIDS which is made worse by the fact that they are so poor, but you are right to the extent that maybe we should be paying more attention to the problem here at home than we are. Obviously, the problem is quite serious here, too, and is not receiving the attention it deserves.

I hope so too. It isnt hard to find good programs out there ..you can find out through your county hospitals and health department about what programs are out there. It takes alittle research and some phone calls but they are out there. The meds are very expensive its true but there are programs that offer help. Without them my son would not be here today. I thank god for the good people in this world who care enough to help rather then judge. My son is not an addict or gay or irresponsible sexually he just happened to be one of the unfortunate ones who got infected. Unfortunaely things happen.

Our church does quite a bit to feed the hungry and homeless.... and we do it out of concern, giving our labor and money. The overwhelming bulk of the money is absorbed by overhead. America lacks good health care as it is based on money and not health. A friend of mine 75 years old uses medicare three times a week over $5,000 per week and he says most is un needed but the doctor can only make money doing this way. AMerica does not need more people insured, it needs the broken down medical help for money removed. Why are doctors from India coming here? They can not do what america allows in other countries.

Here in AZ there is no health coverage help for adults. Young adults frequently do not have jobs with even mimimal benefits. All medical and dental needs have to be paid out of pocket. You can't get past a receptionist without showing proof of insurance or a wad of cash. Most young people I know have to wait until their health problems become severe and life threatening, then they can be seen in an ER. I don't know where you live, but here I can show you hundreds of thousands of people who get turned down for medical care. A good percentage of them with health problems and treatable conditions that will eventually kill them far earlier than necessary- ie, untreated high pressure.
I don't think this is only a failure to treat for HIV. It is however, a good indicator of health care accessibility for young adults. The headline could probably delete the word HIV and still be accurate.
